Eklego is seeking dedicated and compassionate Case Manager to join a team of nursing professionals working in a direct-hire position at one of our Senior Care clients in the greater Buffalo, NY area. This opportunity is 40 hours per week; flexible schedule, may include weekends and evening hours.

What will I do as a Case Manager?

  • Serve as Lead Manager providing operational oversight of residents with the RN Program Manager, and the facility on a daily basis.
  • Works cooperatively with the RN Program Manager to address all day to day resident, family, operational issues which arise.
  • Completes an admission interview(screen) and related paperwork with resident and responsible parties on community screens. Communicates findings to the Administrator and Sales Department on appropriateness of potential candidate and sets up follow up paperwork and move in information as needed.


  • Facilitates resident Individualized Service Plan development and completes all necessary follow up in order to accurately reflect all resident needs and to ensure DOH compliance.


  • Maintains a financial file on each resident from start of admission and ensures compliance with all admission/discharge paperwork and documents meetings and interventions in this file.


  • Responsible for inputting all resident information into Point Click Care and completing service plans, resident face sheets and determining rate information.


  • Responsible for documenting family/provider interventions; complaints and complaint investigations and ensuring follow up by involved parties.


  • Responsible for the management and thorough completion of all Incidents and ensuring follow up to all incidents by nursing personnel. Responsible for sending all required incidents to DOH per regulations and maintaining compliance in this area.


  • Responsible for maintaining all rosters to include the diet roster, emergency roster, special needs roster assigned and distributing them accordingly.


  • Responsible for communicating to families/correspondents and other service providers regularly and assisting the RN Program Manager with any communication/coordination of care.


  • Responsible for coding all check requests and invoices and sending them to billing in a timely manner.


  • Responsible for physical plant communication to the Administrator and Facilities/EVS department and submitting work orders for work needed, including all turnovers needed for timely resident move ins along with  doing daily rounds of the facility to identify needs along with identifying any needs related to temperature control; pest management;  wanderguard, call bell, pagers, delayed egress system  and emergency door system in the building along with other areas.


  • Responsible for monthly completion of invoices for Long Term Care Insurance invoicing to include completion of a cover letter, printing of an invoice and timely submission to the companies.


  • Responsible for providing daily guidance to the Activities Coordinator, RN Program Manager and Staffing Coordinator and communicating needs to the Administrator.


  • Responsible for reviewing the weekly payroll completed by the Office Coordinator before Administrator approval and sending weekly Agency usage to the finance department.


  • Responsible for maintaining the administrative binders with policies, DOH activity, incidents, etc. so they are always survey ready and organized.


  • Participate in resident/family meetings; weekly Rehabilitation meetings; IDT meetings with Home Care; nursing department meetings, upon request; all staff meetings and any assigned campus or community meetings to represent Garden House.


  • Responsible for communicating daily and as often as needed to the Administrator, who is off site, on daily updates, incidents, hospitalizations; family; management and staffing concerns etc.


  • Responsible for the appearance of the front foyer, office areas and overall cleanliness and organization of the building; ensuring Facilities/EVS/Dietary are notified of concerns/requests; Activities keep the building decorated seasonally and maintain this and staff are reminded of areas of concern.


  • Maintains petty cash for the building and completes all necessary paperwork.


  • Assist with periodic observations of meal service and completion of observations with recommendations for improvement.


  • Assist with completion of quarterly QA audits and reporting on results at quarterly meetings.


  • Assist with program and policy development for the program with the Administrator.


  • Responsible for HCS daily checks and assisting any other managers with securing accounts. Back up to the Administrator in an emergency.


  • Responsible for assisting the Administrator with Mutual Aid quarterly printed updates; updating the emergency plan; assisting with full evacuation drills and any other emergencies which may arise.


  • Make any referrals for community services.



  • Assesses, identifies and assists with resident issues which require intervention as to their role, functioning, and interpersonal relationships.



  • Encourages resident participation in daily site activities and works closely with Activities staff regarding the fulfillment of resident likes and interests.


  • Prepares records and reports as necessary and records progress notes in resident records concerning services rendered and management of resident issues.


  • Provides consultation to staff and acts as liaison between staff and residents.


  • Assists residents to maintain family and community ties and to develop new ones.


  • Assists the resident in obtaining and maintaining a primary physician or source of medical care of his/her choice who is responsible for the overall management of the individual's health and mental health needs.


  • Participates in staff orientation development program.


  • Attends all Resident Council Meetings.


  • Attends mandatory in-services as required.


  • Performs other duties as assigned by the Administrator.


Must possess a bachelor's degree from an accredited college or university with major work in human services and one year of full time experience in the provision of services to a dependent population or an associate's degree from an accredited college or university with major work in human services, and three years of full-time experience in the provision of services to a dependent population.  The incumbent of this position is able to understand medical terminology and principles.  Is able to communicate with others of diverse backgrounds and educational levels and is empathetic to the needs of others.
