The Wesley Community is seeking a RN MDS Coordinator to join our team in Saratoga Springs, New York.

The primary purpose of this position is to be responsible for the MDS process (including OBRA as well as OBRA assessments for reimbursement under the Prospective Payment System). The MDS Coordinator performs duties under the supervision of the Director of Nursing.

QUALIFICATIONS

Educational Requirements:

  1. Must have a diploma from an accredited School of Nursing.
  2. Bachelor of Science in Nursing preferred.
  3. RAC-CT Certification Preferred
  4. Current registration with the New York State Board of Regents.

Experience:

  1. Head Nurse, Supervisory and/or Administrative experience.
  2. Geriatric nursing experience.
  3. Extensive knowledge of MDS 3.0.

 

MAJOR DUTIES AND RESPONSIBILITIES:

  1. Act as main reference person for the interpretation of MDS regulations according to the RAI User Manual to the Interdisciplinary Team.
  2. Act as main reference person for the NYS Department of Health for MDS related concerns.
  3. Respond to Medicare appeals as occur.
  4. Assure the completion of the MDS process from scheduling OBRA as well as OMRA assessments (both scheduled and unscheduled) through transmission.
  5. Ongoing monitoring of resident status to identify opportunity for OBRA MDSs (i.e., Sig Change).
  6. Monitor resident condition, participation in therapies and schedule MDS assessments to facilitate casemix. 
  7. Prepare and submit casemix according to NYS Department of Health procedures.
  8. Assist with completion of MDS assessments as needed to maintain compliance and support the nursing department when necessary.
  9. Direct interviews and CAA's to be completed and assist if necessary for completion.
  10. Facilitate weekly Medicare/HMO meetings with nursing/social services/therapy to foster communication re: resident participation/progress in therapy/potential discharge/etc.
  11. Develop a good working relationship with Nursing Administration and maintain constant communication.
  12. Education of Interdisciplinary Team in the MDS process and casemix philosophy and procedures.
  13. Assure the preparation and deliverance of therapy/skilled nursing "cut" notices per Medicare regulations as directed by the Business office.
  14. Assure MDS assessments are completed by the Interdisciplinary Team and transmitted to CMS per regulations.
  15. Prepare PRI's and PASSR screenings as requested by the Admissions office.
  16. Educate and Prepare Nursing Department and other necessary departments for Case Mix twice per year
  17. Educate new nursing staff and necessary departments on how to complete an MDS
  18. Monitor resident participation in therapy daily to facilitate RUG's revenue reimbursements.
  19. Other related responsibilities as assigned.
