Eklego is seeking a dedicated and compassionate Registered Nurse (RN) Clinical Coordinator to join a team of nursing professionals working in a direct-hire position at one of our Senior Care clients in the greater Rochester, NY area. Currently we have clients with Long Term Care and Sub Acute Care/Rehab opportunities, offering a Full-time schedule, for a Registered Nurse Clinical Coordinator for their Transitional Care Unit. This position in a Full-Time opportunity on Evenings, Monday-Friday!

What will I do in this Clinical Coordinator Role?

ESSENTIAL FUNCTIONS:

  • Acts as the nurse in charge who coordinates the daily operations of the unit. Assists the Nurse Manager to coordinate, schedule, assign and audit the clinical operations of the unit.
  • Completes RAPS/Care Planning. Supervises staff to ensure that patient's care plans are followed.
  • Assists primary nurses as needed to transcribe and carry out physician's orders and integrates them into the patient's care plan.
  • Assists in providing direct personal care to patients as needed.
  • Completes FIM Assessments.
  • Assists primary nurses with daily activities as needed.
  • Helps maintain patents at their highest level of functioning by using rehabilitative nursing principles.
  • Assists with therapeutic and diagnostic procedures. Gives medicines and treatments when needed.
  • Coordinates schedules and staff assignments- ensures assignment sheets are prepared daily.
  • Communicates patient's condition by notifying Nurse Manager. Communicates changes in patients' conditions to all staff and families, as condition warrants. Ensures the maintenance of the pert sheet: records in patient's medical records and gives report to on-coming shift supervisor.
  • Assists with admissions and discharges.
  • Audits and ensures daily completion of
    • Wound updates
    • Weekly observations
    • Required diagnostic tests/ procedures (i.e. weights, guaiac's, etc.)
  • Assists with physical day by making rounds with physician and transcribing orders.
  • Completes risk assessments
  • May accept telephone orders by physicians.
  • Supports facility and departmental policies.
  • Maintains patients' confidentiality.
  • Maintains a safe environment, conducive to positive health care and comfort, for patients and staff in accordance with established policies and procedures.
  • Assists in data collection related to residents' care as directed by individual care plans and physician orders and documents per protocol.

REQUIRED CLINICAL SKILLS include but are not limited to:

  • PICC Line certified
  • CPM competent
  • C-Pap/Bi-Pap
  • IV hydration (future)
  • Rectal Tubes
  • Foley Catheters
  • FIM Assessments
  • Wound Care
  • Tube Feedings
  • Ostomy care
  • Suctioning, Oxygen, Trach care

Requires:

  • A valid/current RN License in New York State
  • BSN preferred
  • 3+ years of nursing experience
  • 1+ year of nursing experience in a long-term care or acute care setting
  • Proven assessment skills
  • Strong written and oral communication skills
